There are many different insurance plans available, and the clinic continues to add new insurance plans at various intervals. The most up-to-date way to find out if we accept your insurance plan is to call our office if you are unsure.
When you call to make an appointment, please have your insurance card ready, as we will require information from the card to schedule your appointment. We also require that you bring your card and ID with you to your appointment to help ensure your appointment runs efficiently and without delay.
Medicare
The clinic accepts Medicare. The clinic will prepare and submit your claim to Medicare on your behalf. Medicare will reimburse the clinic directly for its portion, and you will be responsible for your remaining portion along with any deductibles that are not met.
If you have supplemental insurance that we are contracted with, the clinic will also prepare and submit a claim once Medicare has reimbursed their portion. If your supplemental insurance has not paid or does not pay, you are responsible for the balance of the services.
Medicare Advantage
Currently, we are credentialed with some but not all Medicare Advantage plans. It is best to call the office to confirm with us about your plan.
If you are part of a Medicare HMO or Medicare Replacement Plan, we will communicate with the insurance carrier for you in compliance with their plan and network guidelines. You may be required to obtain a prior authorization or referral before your visit with us.
While Medicare Advantage plans are required to provide the same benefits as traditional Medicare, experience has shown that they may delay or deny payment for services. As such, we have found it necessary to request a signed acknowledgment from our patients and request a refundable deposit for services rendered until the insurance carriers submit payment.
Private Insurance
While the clinic is contracted with some insurance carriers, it is not contracted with all carriers and plans. Please call our office for the most up-to-date list of insurance carriers and plans that we accept.
We will prepare and submit an insurance claim for the medical services provided. However, please be aware that you are ultimately responsible for your account, as we may not have a contractual relationship with your insurance provider. If the clinic has not received a payment or determination from your insurance provider, you will be responsible for the services rendered.
Please read all literature provided by your health plan regarding patient responsibilities. Some health plans require you to obtain a referral or prior authorization before the clinic visit. Most plans require a copay and/or deductible payment to be made at the time of service. We are contractually obligated to collect your copay prior to your visit with the doctor.
If you are undergoing a procedure, your insurance company may require that you pay two copays: one for the doctor’s service and one for the surgery center or supplies. We do not have control over this policy. It is important for you to learn about the benefits of your insurance plan so that you may make the most beneficial choices for your health care needs.
No Insurance
We accept patients that are self-pay and provide the same quality personalized care that meets your eye health needs. In cases of financial difficulty, please let the clinic know, as we may be able to assist in obtaining alternate financing through charitable organizations or financing programs.
